Practice Nurses in General Practice
Our Practice Nurses are integral members of our team.
Our Nurses provide comprehensive nursing care for our practice working alongside our General Practitioners to deliver:
- Childhood & Travel Vaccinations
- Wound Care
- Patient Education
- Health Assessments
- Chronic Disease Management -including nurse led clinics
- Providing up to date information on health issues
Working closely with both patients and Doctors, our Practice Nurses work to ensure all care is well coordinated across all health disciplines.
Our Practice Nurses are actively involved in ongoing continuing education to ensure their clinical skills and knowledge is always up to date and that the information and treatment we give to our patients is current relevant and best practice.
An initiative for patients with more complex medical conditions. Our nurses run Chronic Disease Management Clinics in conjunction with the patient’s doctor to assist patients to develop healthy living plans. The plans outline all aspects of their care and look at improving health outcomes with the assistance of a Care Team including the GP and other allied health specialists. Talk to your GP or the nurses for further information.
A comprehensive overall assessment for Patients over 75 years of age. Health Assessments are an important opportunity to take a preventative and pro-active approach to health care. The assessment normally takes 45-60 minutes. The health assessment can be done here at the clinic or at home.
